Doula Pre-Approval Form | Doula Invoice | Travel Form – Please review the information below.

Doulas contribute to empowering birthing parents to feel more in control and secure about their birth experience, which promotes a stronger bond between parent(s) and newborn. Doula support can improve health outcomes for Indigenous families by bringing the birthing process closer to home.

In order to support Indigenous families through the DAFGP, doulas must:

  1. Submit completed Doula Pre-Approval Form and allow up to two (2) weeks for processing and approval.
  2. Submit completed Family Registration Form and allow up to two (2) weeks for processing and approval.
  3. Submit one invoice per pregnancy within two (2) weeks of completing service.
Doula Pre-Approval Requirements

– Copy of Birth doula training certificate and/or Copy of Post-partum doula training certificate
– Copy of DSA Web Referral List membership and/or Copy of DONA International Certification

– Copy of “other” certificates (breast feeding training, infant CPR, etc.)
– Copy of cultural safety training certification
